DOI: 3/5/1998. The patient is a 62-year old male material handler/breaker who sustained a work-related injury to his low back from lifting heavy objects. As per OMNI entry, the patient is status post L4-5 discectomy. His future medical care includes office visits, non-steroidal anti-inflammatory drugs, analgesics, physical therapy up to 12 times a year, and epidural steroid injection, with the possibility of surgery.
