PT is an 18-year-old, Caucasian male from C***, T****. PT stated the reason he came into treatment was due to his DUI and being arrested for having pills without a prescription. He stated after he was released from jail he wanted to seek treatment and his attorney supported him on this decision. He reports his first use of opiates occurred at age 17 when he used oxycodone. He states that he was using opiates to treat his chronic back pain.

● Currently in the 12th grade and currently not employed
● Currently reports not being in a relationship
● SES is unknown; however, lower-middle class estimated AEB his father source of income through Social Security
