On 6/28/16 I spoke with Ms. Volk. Ms. Volk said she is postponing her epidural injection to the fall now. At this time she has no further appointments. Due to the inactivity of this file I am closing with the approval of Ms. Blodgett.

SUMMARY

Ms. Volk said she was a restrained driver involved in a head on collision while driving on M-25. She stated the other vehicle lost control and crossed over the median and hit her vehicle head-on. She stated she was also rear-ended immediately. She denied any loss of consciousness. She felt immediate pain in her lower back and right ankle. She was taken to McLaren Bay City via ambulance and then transferred to Hurley Hospital due to the extent of her injuries.
