At the time of the assessment Ms. Waddell reports she is currently having thoughts of harming herself. She expressed feelings of depression as stressor contributing to her distress. She describes feelings as: feelings of hopelessnes, worthlessness, guilt, isolation, insomnia, tearfulness, and loss in usual pleasures. Ms. Waddell has a history of chronic mental health and substance abuse issues. She states, "I don't know what to do now that I'm out, when I was home I was okay but then I got depressed and want to kill myself." Ms. Waddell reports attending several inpatient hospitalizations and substance abuse treatment programs. She believe over the course of her multiple hospitalizations there has been some progress She has a history of multiple suicide attempts.
