Provide two examples, with supporting evidence, of an improvement in patient safety that resulted from clinical nurses’ involvement in the evaluation of patient safety data at the unit level. Supporting evidence must be submitted in the form of a graph with a data table that clearly displays the data.

Example 1 Neonatal Intensive Care Unit interprofessional infection prevention team improves patient safety by eliminating neonatal Central Line Associated Blood Stream infections.

Background/Problem
St. Mary’s Neonatal Intensive Care Unit (NICU) Interprofessional Infection Prevention Team was formed to review NICU CLABSI data in an effort to eliminate CLABSI. This team includes nurse leaders, clinical nurses, providers, hospital infection prevention nurse, and a pharmacist. The team reviewed the policy and procedures regarding central line management and provided education focused on best practices to reduce CLABSI. Despite reeducation and reinforcing central line management, evidence based procedure standards for insertion, fluid change, proper technique for obtaining blood cultures, and regularly scheduled dressing changes, CLABSIs still occurred.

They identified that they were often able to reach zero CLABSIs, but unable to maintain and sustain zero. The team concluded that the interventions they had put into place were not sustaining zero CLABSI.
Goal Statement(s)
In 2015, NICU Interprofessional Infection Prevention Team reestablished the goal to eliminate CLABSI and sustain zero CLABSIs in the NICU.
Description of the Intervention/Initiative/Activity (ies)

The team realized that in order to sustain zero infections, central line utilization rate would need to be reduced to further reduce CLABSI

Infections are a common risk factor for any patient in the hospital. The occurrence of an infection is an even greater concern when the patient receives an invasive procedure, such as placement of a Central Venous Access Device. With proper technique and licensed staff placing a central venous access device, the risk for complications can be minimized. Champions for Central Line Care is a journal that explains “the team approach for reducing CLABSIs. (Reed, Brock, & Anderson, 2014, p. 40)” CLABSI can be defined as a Central Line-Associated Blood Stream Infection (Giddens, 2014, p. 469).…

One key element of safe nursing practice, as written in TJC Patient Safety Goals, is NPSG.01.01.01. This is that we always identify the patient in two ways. This could be done by asking them to state their full name and date of birth. This ensure that the patient is getting the appropriate care, medicine, and treatment (NPSG). It is extremely important to do this right away when walking into a patient’s room, so we know that we are explaining assessments, procedures, and providing education to the right person.…

Central line associated bloodstream infections (CLABSI's) are one of the most deadly healthcare associated infections (HAI's). Approximately 12-25% of CLABSI's result in death and the average cost per case is about $26,000. In recent years, between 2001 and 2009, CLABSI incidences have decreased from 43,000 to 18,000. This has saved the health care industry 3-6,000 lives and approximately $414 million in ICU's in 2009 alone. To this day, many hospitals are now reporting zero CLABSI's.…

As a student nurse I require second development in infection control, so I can contribute to safe practice and decrease the spread of infections in nursing care. Patients in the health care settings have an increased risk of acquiring infections due to nosocomial infections which is also known as health care-associated infection. This infection occurs after admission to a health care facility due to transmission of microorganisms (Potter & Perry, 2014, p. 624). “Clients are becoming more difficult to treat. Health care- associated infections results in significant morbidity, mortality, and cost” (Sydnor & Perl, 2011).…

Infection Control Risk Assessment (ICRA) In this world, humans are often oblivious to pathogens in the workplace, including diseases and illnesses that could kill hundreds of people every day. One often unthought-of risky setting would be health care facilities when they are in their very first stages, the construction stage. Nevertheless, this stage in time becomes a critical point to stop most infections from beginning in the first place, and if done right will keep later patients from becoming sick from a nosocomial infection.…

Applying strict protocols for contact, droplet, airborne, and isolation precautions in place limits the spread of nosocomial infections. Gloves, masks, and gowns should be available to every person on staff in a healthcare setting, not just the nurses. Agencies at the federal and state level, as well as accredited healthcare organizations require an infection control specialist. This is “a person or persons [who are] designated as infections control officer or officers to develop and implement policies governing control of infections and communicable diseases” (Bryant, et. al.…

Hospital acquired infections are particularly challenging because many of these infections have developed antibiotic resistance (Pollack, 2010; Rosenberg, 2015). However, most hospital acquired infections are preventable through a variety of techniques, including the use of hand washing, equipment sterilization, use of gloves, sanitizing surfaces, among other things (Mehta et al., 2014; Monistrol, Calbo, Riera, Nicolas, Font, Freixas, & Garau, 2012; U.S Department of Health and Human Services, 2015). The key here is consistency, and a hospital culture committed to reducing and even eliminating hospital acquired…
