point) Interpret A.S.’s laboratory test results and describe their significance to the patient’s disease process. Both the Hemoglobin and hematocrit are low which can be a sign of kidney failure. When the kidneys start to fail the amount of erythopoietin being relased c often times decreases and the erythopoietin is what stimulates the red bloos cell production. WBC’s are on the low side but still within normal range.…
At approximately 8:45 a.m., Mr. Rossignol and the plaintiff were driving their respective vehicles in the northbound lanes of I-294/94/80 and just west of the Illinois/Indiana state line. At that location, traffic in the northbound lanes of the highway is actually traveling west. The interstate at that location has four lanes in each direction and the northbound and southbound are divided by a median with a vertical concrete barrier. Mr. Rossignol was driving in the second lane from the median barrier.…
The Joint Commission has instituted a number of goals nationally with the aim to improve patients’ safety. The goals selected look at areas that are of concern in the healthcare industry, particularly how it affect patients’ safety and make recommendations how to reduce if not eradicated these. Using the evidence-based prevention measures instituted by the Joint Commission both hospitals, Holy Cross Hospital located in Silver Spring, Maryland and Shady Grove Hospital situated in, Rockville, Maryland received accreditation, and report is available. A detailed analysis will look at the scores received by both institutions and analyzed what this says about the quality of care these institutions can and will provide. In addition, how these institutions…
He is not using the spinal cord stimulator (SCS) due to the battery heating up. He has tried to use ice as directed by Boston Scientific. They think that since he has lost so much weight that it may need to be tacked down. His tailbone is very painful and he changes positions frequently, he has had an epidural or trigger point injection before to the area which only lasted maybe 2 months. Dr. Rampersaud will do another injection on 9/11/17.…

Premise: The Nursing Action-plan on the use and sustainability of Barcode Medication Administration Problem Statement According to Aiken et al. (2012), the concern regarding patients’ safety is a global issue and not concentrated to particular hospitals. The study by Van Den Bos et al. (2011) describes some of the means through which patients could be injured such as ineffectual sharing of information among clinicians, delayed treatment, and sometimes misapplication of health technology just to mention a few.…
Preventing Falls in Acute Rehab Unit: Safe Patient Discharge is a Priority Introduction of Concept Preventing falls of all patients is the first priority of acute rehab unit. Stroke and traumatic brain injury patients are at higher risk due to cognitive impairment and weakness on the affected side after the stroke and fall. Patients forget their limitation of mobility and try to move independently without realizing their weakness after the stroke and ends up in falling and suffer more injury. Patients’ safety is the highest priority. Joint Commission’s National Patient Safety Goal 2009, ‘Fall Reduction Program’ (2014) stated every health care organization’s fall prevention program is expected to include all patient care settings and populations.…

Based on the information provided, a comprehensive assessment of Sarah’s speech, cognition, and orientation would be necessary. In order to properly assess her condition, it would be beneficial to know how recent her injury was and how long she was unconscious post-injury, as this can alter the prognosis of her condition. Assumedly, the Glasgow Coma Scale was completed on Sarah when paramedics first arrived on scene, and would be inappropriate to complete after surgery, as the anesthetic could skew results; however, I would complete the Rancho Los Amigos scale, had it not be completed prior to a potential MRI. Because Sarah is currently in surgery, it would be assumed that she had also already had an MRI and these scales had already been completed.…
SECURITY CONCERN ON HIGH VOLTAGE MEDICAL RECORDS BEFORE A MOVE VITAL REQUIREMENTS OF MEDICAL RECORDS DURING A MOVE When moving medical related files, it’s necessary to know the hefty fines involved in case a privacy breach takes place. That means penalties of up to $ 1,500, according to updates on the HIPAA (Health Insurance Policy and Accountability Act.) Compacting The Identity Theft During A Moving Exercise…
Prior to the discovery of X-rays, the diagnosis of spinal injury was primarily based on clinical observations. Because of the impressive clinical presentation in spinal injury patients with concomitant neurological deficits, spinal injuries were frequently categorized as injuries with or without spinal cord injury (SCI) . Bohler published the first categorization of spinal injuries in 1929 based on plain radiographic examinations of treated spinal injury patients during World War I14. Since then, there has been an enormous growth of injury classification systems in spinal trauma literature, ranging from general spinal injury classifications to specific classifications.…
Introduction: In recent years, the goal of healthcare has shifted. Not so long ago, the sole treatment of acute or chronic illness was the focus of the healthcare field. This can be referred to as a tertiary prevention technique (AMFC…..). In practice, tertiary prevention includes the rehabilitation of a patient after an acute illness or injury.…
Prevention of Inpatient Falls Patient falls remain the most common adverse event in acute care facilities, with 2%-15% of hospitalized patients reported to fall at least once. Falls can lead to pain, loss of function, fear of further falls and even death (Tanaka, Sakuma, Ohtani, Toshiro, Matsumura, & Morimoto, 2012). An increased focus is being placed on inpatient falls because of morbidity, mortality, increased cost of care, and lack of reimbursement (Cumbler, Simpson, Rosenthal, & Likosky, 2013). The National Database of Nursing Quality Indicators (NDNQI) defines a fall as “an unplanned descent to the floor with or without injury to the patient” (Miake-Lye, Hempel, Ganz, & Shekelle, 2013).…
