George is a third grader at Charlotte Elementary School (CES). CES serves students kindergarten through fifth grade. George is currently in a general education classroom with twenty-two additional students. For the past 3 years, George has been in a sub-separate classroom. To ease his transition into a general education setting, George works a 1:1 aide. George comes to class everyday prepared and ready to learn. He enjoys working with his peers and working with technology. Last year, George performed in the average and below average range. At the beginning of third grade, George was reading at grade level and increasing his independence. However, since February, his teachers have seen a decrease in classwork production. His family and friends call him the “Computer Wiz”. George also enjoys created jingles for commercials for this future business he plans on opening. George’s favorite place to visit is Disney World to see Mickey Mouse.

Last year, George performed average in math. He naturally takes to numbers and enjoys solving equations. His second grade teacher stated that George looked forward to math everyday and that math was by far his favorite subject. Though George didn’t volunteer himself, if he was called on he would provide his answer. This year, George’s feelings toward math haven’t changed. George is doing exceptionally well and his teacher is happy with his progress. The only area of concern is word problems. He can solve a majority of the problems correctly, but often needs his 1:1 for assistance.

At the beginning of the year, George was performing at grade level in ELA. As the year has progressed on, he seems to be losing his focus and attention. George was an outstanding speller and was scoring at the top of his class each week. After winter break, his teacher began to notice a decrease in his weekly scores.
