This assignment will discuss about a 49 year old, woman who was diagnosed with Asthma. A real-life experience as a main source of information discovers the discomforts and expectations of the Long Term Conditions (LTC). Looking at her self-management plan and the self-management support she received to manage her health and improve her quality of life. The importance of health education and services are provided to manage the complications and maintain a good quality life. For this assignment, I had to build the trust, to obtain all the relevant information. I was invited into her home and this was her decision as she felt more comfortable. The impact of asthma is different for each individual. Tu’s asthmatic symptoms’ such as chronic cough, wheeze, chest tightness and shortness of breath has impact her a lot. In her young days she used to miss school and when she started work as a healthcare she reported sick a lot. After few months of her employment she had an asthmatic attack. This was a life threatening experience for Tu, which lead to an acute hospital admission for 3 weeks. She was admitted in acute medical ward, which was 15 kilometres away from where she …show more content…
In this case I as a registered nurse will support Tu by, improving her health literacy. I will be providing her with all the relevant information’s regarding asthma, in a way that Tu can understand. This will help her make informed decisions and set goals for herself. She will be able to giving information about her health to the doctors on routine visits and will be able to ask questions as well. Secondly will develop an action plan and set goals with Tu, in consideration with her preference and priorities. What Tu can do in her home setting that will avoid her triggers and she will be able to maintain healthy behaviour. Setting goals that are proactive and flexible, which will fit Tu and her whanau and is culturally appropriate as well. This will help release her stress, which is one of her triggers. Thirdly will engage Tu to have a healing relationship with health care team which will enable her to sustain regular follow up. Following up with Tu regarding her participation in self-management programmes is essential for her to have a better quality and meaning full
