CCIB received a complaint via phone call from RP stating that his father JD had pressure sores on his big toes. RP stated the first time Hospice notice JD had a pressure sore was on 4/5/2017. RP stated on 4/5/2017 JD hospice nurse Dawn Knight observed a pick spot on his left big toe and instructed Susan (Facility Wellness Director)to put warm compresses on the toe to help circulation. RP reported that after being instructed staff never proceeded with the compresses. RP stated on 4/8/2017, his mom Jeanne was visiting JD and she asked “what’s wrong” and JD replied “his toe hurts”, so she took off his shoe and observed his toe exploded in his shoe. RP stated there was puss and blood in his shoe. RP reported that he contacted Dawn to come to the facility to look at …show more content…
RP stated on 5/15/2017, he received a call from the Dawn stating that JD has another pressure sore on his right big toe now. On 5/16/2017, RP stated that JD was to be moved to his new facility (Adalanto Silver Heights). However, RP reported that did not happen because while in the process of moving JD his right toe exploded. RP reported blood and puss was all over the floor and on JD’s bed. RP also stated Jeanne was also at the facility while the movers were trying to move JD, and then when JD toe exploded she ask staff to help bandage his toe again staff “they don’t have any medical staff to help”. RP stated that he called Dawn to come to the facility to help bandage JD, which took about 2 ½ hours until Dawn got to the facility to help. RP reported at this time the moving staff had left, so JD had to stay another day at the facility. RP stated that JD was finally moved as of 5/17/2017 to his new facility. RP mentioned he has concerns about the other residents and the facility not being trained
