This worker Naisha Lawrence was able to meet with Dora Morales at her home located at 69 Bloomfield Avenue, Paterson New Jersey. The purpose of the visit was to meet Dora Morales, Manuel Rosado and their son Nathan Rosado for the month of February. Additionally, the worker was to conduct a family team meeting with Ms. Morales and Mr. Rosado supports in order to discuss the family's long term view and what needs to happen in order for the case to move towards termination. Lastly, the worker was to obtain Nathan and Manuel Rosado's social security number in order to complete the request for 4c's funding as well as to run a background check on Mr. Rosado.

J. Rosado told the worker that she would be in her grandson life and he will make sure he is good. Ms. E. Vasquez also stated that the baby could live with her anytime. She stated that she loves Nathan. The family reassured the worker that the child would be taking care of no matter what happens between Manuel and Dora.

The worker was able to recap the plan. The worker stated that Ms. E. Vasquez will babysit Nathan while Ms. Morales goes to options counseling. The worker stated that Ms. Morales will go to options counseling on 3/6/2017 to get reenrolled into treatment. The worker shared with the family that she will obtain information on how Ms. E. Vasquez can become a 4 c’s provided. The worker stated that she would inquire about the status of Ms. Morale’s benefits with TRA.

The worker asked the family how she would know this plan is worker. Ms. Morales stated when things are in place.

The worker stated that she will follow up with Ms. Morales to ensure that she is attending treatment. The worker encouraged the family to also hold the worker to what she said she would do. The family agreed. The family agreed to do so.

The worker asked what would be the back up plan if this plans does not worker. Ms. E. Vasquez stated that the baby would be with her.
