I: CM assessed client's mental, emotional and physical state. CM used open ended questions to inquired about housing updates.CM asked questions regarding the client’s monthly payment from SS and SSDI. CM assisted client with ordering a new debit card in order to setup direct deposit for SS and SSDI payment.
R: Veteran was seen properly groomed and dressed appropriately wearing a blue shirt and blue jeans. He maintained appropriate eye contact and engaged in the meeting. His affect was appropriate and mood appeared euthymic. Client reported having trouble setting up direct deposit for SSDI payment.
