G: Met with Client for weekly session and to discuss his housing goals.

I: CM assessed client's mental, emotional and physical state. CM used open ended questions to inquired about housing updates.CM asked questions regarding the client’s monthly payment from SS and SSDI. CM assisted client with ordering a new debit card in order to setup direct deposit for SS and SSDI payment.

R: Veteran was seen properly groomed and dressed appropriately wearing a blue shirt and blue jeans. He maintained appropriate eye contact and engaged in the meeting. His affect was appropriate and mood appeared euthymic. Client reported having trouble setting up direct deposit for SSDI payment. Client agreed to notify CM when he receives his new debit card in order to received assistance with enrollment for direct deposit.

The client reported having all of his documents ready for his next housing appointment as requested by Laurie Garza. The client reported his plan to walk to his next appointment with LAMP and to reserve lunch for that day.

Substance abuse: Client reported abstaining from using illicit substances. Client reported his intention to remain sober while in the program.

Mental Health: Client denied having any PTSD symptoms at this time. The client reported being in “good spirits” and denied S/I, H/I.

Financial: client reported having SS and SSDI payment issues for over a month. The client reported issues with mail delivery and identity theft as the reasons why he hasn’t received his monthly check. The client finally agreed to setup direct deposit by obtaining netspand debit card with the help of CM. Client receives SS in the amount of $540 and SSI in the amount of $369.40 a
