She is the 1st member of health team that the patient will contact hence will be the person to start planning of her care.
The nurse as a coordinator in health team start by assessing patient health, Do vital sign and invite other health team professional, she will be the one to collate laboratory results make sure patient is review with the result and medical diagnosis is made, then make nursing diagnosis set target goal, plan care implement and evaluate it and when patient is eventually improve with cares provided, she is the one that prepared patient for discharge, coordinate concerned member of health team for continuity of care at home, and also keeping necessary clinic appointed
