Pressure Ulcer
Introduction:
Pressure ulcers are the pressure points in the body which can cause ulcers if not been monitored while the patient is on bedrest, There's types of symptoms that is caused by it which are related to it in this assignment that has been made it will be conversing about pressure ulcers and it's risk factor's and what kind of patients that are more easily exposed to the symptom's & the intervention's that can be providing by nurses.

The barden scale takes the necessary measurements to insure that the patient that is on bedrest is safe from developing pressures ulcers involving many other things.There are risk factors that causes the pressure ulcers. The first factor is the intrinsic factor. It involves the skins reaction towards the pressure ulcer, Age, Weight, Disease , Mobility, Incontinence, Nutrition The second risk factor is the extrinsic is the outside factors that effect the tolerance of the skin such as, Shearing, Friction …show more content…
Each sections has 4 sections except one that has 3 section, The sensory perception, Moisture, Activity, Mobility, Nutrition they all have 4 sections but the friction and shear has only three. The scale is used in changes of shifts or every 24 and fixed based on the patient well-being.
Nursing Interventions: The nurse is going to asses in paying close attention to the area's that is more vulnerable to pressure ulcers, Sores or craters. Therefore, The position should be changed every 2 hours, Use pillow cases or foam paddings, Powders from medical stores to help decrease the sores, Use creams prescribed from doctors, Have enough nutrition to nurture the body, Drink water 8 - 10 cups a day to keep it moist, The skin should be dry and clean cause if it has germs that will be more at risk to sores.

In conclusion what has been established in this assignment is the meaning of pressures ulcers and how its causes and the precise tools that are used in pressure ulcers and interventions that are made. Therefore it should be well taken care of. From the assignment and the research that was done that it’s a very sensitive subject
