One of the most important aspects of discharge planning is making the arrangements for continuity of care, which can include follow-up appointments, referrals to out-patient therapists and services, medication plans and prescriptions, and other arrangements. A good discharge planning may be one strategy to protect against early readmission for psychiatric patients because it provides stability of the patient’s medical condition, and prepares the patients emotionally and practically cope in the community, with appropriate aftercare supports. Patients with a history of repeated admission are most at risk and require special efforts to break the readmission cycle. Understanding what helps the patients to stay out of the hospital is the purpose of this study. With Effective discharge planning there is continuity of care for individuals living with mental illness which reduces suicide and hospital readmissions rates as well as improves the patient’s social functioning and quality of life for the patient). A smooth discharge to community requires close cooperation among individuals with mental health/psychiatric problems, their families, and community-based agencies.
