Patient Assessment

I gave my presentation to a nursing student from the University of Saint Catherine. Her name is Cathy Lee and she is a 3rd year going to her 4th year. I made an appointment with her on the Saturday the 25th of November at the University of Saint Catherine lounge. Her patient care role as a future register nurse is to assess patient’s health status and monitor how patient is responding to their care plan. As a register nurse student, Cathy provide care around the clock during rotation by observing and direct interaction with patients. Cathy is also responsible for following physician's order. As a nurse, she is responsible for evaluating the patient care plan before starting any therapy.

In comparison to patient care process in pharmaceutical care, nurses will perform diagnostic observation of the patient health Instead of going over medication and drug related needs. Nurse also make clinical judgement about the client response to health concerns and is a difference compare to use finding indication of drug therapy problem. One example is asking the patient to reflect on their pain using the pain scale of 1-10 with 10 being most painful and 1 being least painful. Others include stress reflection, poor nutrition and or other related to indication. Base on these evaluation, nurse will develop a care plan base on measurable assessment. Two goals of therapy will be implemented with one being a short-term goal and the other being a long-term goal which is similar to patient care process goals of therapy format. Goals of therapy and care plan is then documented so other professional can have access to it. Nurse care plan also include a follow-up evaluation on the patient’s status to see if the care plan is effective and if modification is needed. Elaborate in detail on pharmaceutical care as I learn more in the next few years in pharmacy school and how it is different and continuously changing. In the future, a video recording of the presentation Like voice tread that was conducted in Becoming a Pharmacist will be great.
