This clinic serves diverse population that are mostly non-native, immigrant and refugee majority of whom requiring interpreters. She appreciates the work that the public and community health providers do because of the difference they make in the underserved populations in their community which makes access to care easier regardless of language barrier, lack of insurance or affordability. This was a patient that I wrote about in my differential diagnosis paper. After she was prescribed low dose anitdepresants, and ENT treated her recurrent sinus infection and DSH transferred her to school closer to home with our recommendation, she is now migraine free. In fact, I saw her 2/28 for well-women exam, she taking her Amitriptyline 10mg PO nightly, she is going to school and she seemed happy overall. I also saw her twin boys who were 14 years old for well-child checks. Per report, the boys were health, no previous medical issue, they were thriving in school and adjusting to their new surrounding and their mother reported no concern. Upon chart review, one of the twins had high blood pressure and during physical examination both twins were noted to have grade I and II cardiac murmur heard during systole on the second intercostal space on the right and left midclavicular line which the mother was never aware. Both boys and their mother reported no previous hypertension or cardiac issue. I reported my finding to my FNP preceptor Cindy Cho. While my suspicion of the murmur is innocent, we nonetheless referred the kids to pediatric cardiologist for further work up and treatment if needed. Features of innocent murmurs are: No family history of murmur, normal pregnancy/prenatal care, no physiological symptoms and the child is older than 2
