The interviews were then audiotaped and transcribed verbatim. After each interview or focus group the research team analyzed the data and subsequent questions were developed. Data analysis progressed through three specific phases including: axial coding, open coding, and selective coding. Throughout the entire analysis process, conceptual diagrams regarding the transitional process were revised constantly. In order to determine the appropriate sample size, several factors were considered. These factors included important data elements such as quality, scope and the complexity of the data. The transferring of patents to skilled nursing facilities is a task done by many discharge nurses in hospitals. Due to the findings seen in this study, further research needs to be conducted to see if the same results are found in a larger sample size that encompasses more than one state. If further research was done with a large sample size across the country, then the research subject could essentially be seen as a global issue. This study expresses the need for better communication among nursing facilities, which is a vital skill for nurses to have. Some recommendations based on this article that nurses should take into consideration is the importance of communicating all patient information to SNFs in a logical, understandable and clear manner. Without clear communication, nurses can expect to encounter obstacles that can delay care, threaten the safety of the patient, and result in patient and family
