SC received phone call from Pa’s informal who reported that the Pa was hospitalized due to a fall. The Pa was admitted at Penn Presbyterian Medical Center from 3/29/2016-3/31/2016. The Pa was treated and discharged injures to tail bone and bottom. The Pa is frail, severe dementia, suffers from ambulatory dysfunction. SC recommends and Pa’s informal primary caregiver is requesting temporary increase 7 hours M-F 7:30 -2:30 assist with morning and afternoon routine including bathing, dressing, grooming, incontinence care, and meal prep, until May 1, 2016. SC reviewed ISP and 10 point check list including cons rights and responsibilities.
