Prilosec Case Study

Prilosec (omeprazole), a proton pump inhibitor that reduces the amount of acid produced in the stomach in conditions caused by excess stomach acid. Prilosec suppresses gastric acid secretion by target inhibition of the H+/K+ ATPase enzyme system at the secretory surface of the gastric parietal cell. This enzyme system is considered as the acid (proton) pump within the gastric mucosa, Prilosec has been characterized as a gastric acid-pump inhibitor, because it blocks the final step of acid production. This result is dose-related and leads to inhibition of both basal and gastric acid secretion regardless of stimuli. He states that his signs of GERD have resolved after starting therapy four weeks ago. Common side effects of Prilosec include fever, cold symptoms such as stuffy nose, sneezing, sore throat, stomach pain, gas, nausea, vomiting, mild diarrhea, or headache. L.G.’s new onset of abdominal pain and vomiting could be related to Prilosec since it was started with the initiation of this medication. Therefore, the dosage should be reduced and the patient should follow-up in two weeks to evaluate if the symptoms have resolved, worsened, or has remained the same. Prilosec has many potential drug interactions. These drugs include bosentan, cilostazol, clopidogrel, cyclosporine, diazepam, digoxin, disulfiram, iron, methotrexate, St. John's wort, tacrolimus, antibiotics, antifungals, antivirals, warfarin, diuretics, and anticonvulsants. Currently, L.G. is not taking any other medications; therefore he is not presently at risk for drug interaction effects. However, since he has a history excess stomach acid, he is at risk for H. Pylori infections. In the case of an H. pylori infection, he must be prescribed antibiotics with caution since drug interactions with Prilosec are
