On or about Tuesday, October 13, 2015 at 0332 hours your AFFIANT (Patrolman Joshua Pastucka) was sitting stationary in the Unity Cafe parking lot located on the 400 block of E. Centre St. (State Route 54). Your AFFIANT observed a red Chevrolet Blazer traveling South on N. Union St. fail to stop for the stop at the intersection of N. Union St. and E. Centre St. The Blazer after failing to stop continued to turn left onto E. Centre St. Your AFFIANT observed the registration plate lighting on the Blazer was on working. Your AFFIANT observed the Blazer make a left turn onto N. Bridge St.

Your AFFIANT exited the Unity Cafe parking lot and got behind the Blazer on N. Bridge St. where your AFFIANT activated emergency lights and conducted a vehicle stop on the

O'DONNELL was anxious, restless, and agitated with fast irritated speech. Other indicators your AFFIANT observed were jaw clenching and licking of her lips, popping her lips and tongue like a lack of saliva (cotton mouth), and facial tremors.

Patrolman David STAMETS of West Mahanoy Township Police was on scene and assisted your AFFIANT. STAMETS spoke with O'DONNELL as well and observed several indicators of O'DONNELL being under the influence of a controlled substance. STAMETS asked O'DONNELL to perform a variety of field sobriety tests. O'DONNELL unsuccessfully completed the field sobriety tests. Speaking with O'DONNELL after the field. sobriety tests O'DONNELL admitted she was just recently smoking bath salts prior to leaving and getting stopped by police.

O'DONNELL was taken into custody and requested to submit to chemical test of blood. Your AFFIANT read the Penn Dot DL-26 form Chemical Testing Warnings to O'DONNELL. O'DONNELL gave consent to submit to the chemical test. O'DONNELL was transported to Schuylkill Medical Center South where her blood was
