Physical assessment should had been number one priority for any oncoming nurse after shift change. From simply interacting with the parent about the child health status, would give indication of sign and symptoms of dehydration. No one knows their child better than their mother. If the mother has any concerns about their child status, it’s our responsibility to pay attention to every details and to do a thorough head to toe assessment. The mother can answer to questions such as fluid and food intake, number of wet diapers, mental status, drowsiness, and even irritability.
