Both Officers arrived at 0148 hrs. and met with Nurse Christiane Dotson who stated that the patient, Sanchez-Garlobo, Manuel (MRN: 70032631/FIN: 84678786) was belligerent and would not follow any instructions given to him by the attending Physician or any nursing staff. She also stated that he had a pair of scissors in one of his pockets and it was taken from him because of the way that he was acting. On Tuesday 08/22/17 at 1207 hours Jose Guerrero reported he had follow up information for this case to include video surveillance and hospital reports. I contacted Guerrero at his business, JG Tires, located at 810 C St SE. Guerrero provided me with paperwork from Valley Medical Center and a personal thumb drive which he stated had a copy of the video footage from the initial assault. Anonymous staff thinks that resident Nola Gucliano did not die of natural cause; RP believes that her death was due to the improper care and medications being administered to her without orders. Around 8/15/17, RP noticed that staff signatures are being forged on the controlled substance page and the entire page was "redone". RP stated that medication bottles are being stripped of their labels and the medication is being given to other residents who do not have the medication prescribed to them. On Wednesday, November 16, 2016 we attended the Missouri State Board of Nursing to watch six nurses come forward to defend their license against the council. The six nurses that are reviewed by the board, had violations and disciplinary actions to be taken. The first nurse who was a registered nurse had disciplinary actions against her license and was practicing in the state of Arkansas without a renewed license. The House Select Investigative Panel on Infant Lives has issued a subpoena ordering Dr. LeRoy Carhart to answer questions regarding botched late-term abortions at his Maryland clinic. He allegedly sent at least five women to hospital within a span of about five months due to complications after abortive attempts. Carhart's clinic, Germantown Reproductive Health Services (GRHS), is one of the few abortion facilities in the United States that performs third-trimester abortions. The women had been placed on probation for unable to practice safely by reason of alcohol or other substance abuse. Ms. Owens was reported by a co-worker to have a slurred speech, was confusing co-workers’ names, appeared disoriented, and staggered while walking. The DON was called and came into the facility where Ms. Owens was working to observe her and then requested she submit a drug test where at that time it showed up positive for benzodiazepines and opiates. A sickle cell client who was admitted was placed on a Patient Controlled Analgesic (PCA) pump. The suspected that the client had tampered with the pump and had received more medication than was ordered by the physician. The tampering could lead to an opioid overdose and would have legal ramification for the hospital. Once it was determined that the patient was tampering with the pump, it was removed and sent to engineering for analysis.
