CHIEF COMPLAINT: Re-evaluation initiated; however, patient being discharged.

Patient has no showed for the last two weeks of speech therapy. SLP met him at his room today and he stated he did not want to do speech therapy any longer. MD was informed and he was discharged.

Inmate had just received new hearing aids and had demonstrated marked improvements in overall communication ability by just adding the hearing aids. However, staff reports and patient reports that by increasing the overall loudness, he does not tolerate wearing them for very long. Nursing was encouraged to help him put them on for a few hours in the morning, allow him to take them out and put them back on a few hours in the afternoon. He will need some …show more content…
Overall word fluency was severely impaired. He is unable to list a name of items in categories. He is unable to listen to a list of words and determine the category that they are in; however, he could fill in the blanks with phrases such as, the grass is "green." Auditory processing and retention was generally within functional limits. He was able to listen to a short paragraph and answer yes/no questions correctly, was also able to listen to a variety of sentences and recall them with only a mild to moderate deficit. In general, he still struggles with recalling daily events and explaining actions within pictures. His general response is, "I don't know," when asked an open-ended question. It is suspected that his reasoning abilities would also be impaired such as describing how two items are alike, how they are different, etc. Overall, without the use of his hearing aid inmate had made improvements; however, was beginning to plateau. He was using the strategy such as easy onset and carrier phrases such as, I want or I need or I have to initiate his speech to help him elicit his wants and needs. Due to consecutive number of no-shows and patient verbally requested to be discharged, the patient will be discharged from speech therapy at this time. Provider has been informed. Please re-refer to speech therapy in a few months if patient is interested in returning to improve overall communication abilities as he will have marked difficulties when leaving the present
