1. What did you observe during the patient interactions with others? What does this tell you about how the family and/or patient is coping with this hospitalization? Throughout my shift, none of the patient’s family members visited her. She is a single mom and I do not think she has a strong support system. Lack of strong support system may affect her recovery. In addition, this is her second hospitalization in the month of August.

2. What specific approaches did you use to develop a therapeutic relationship with your patient?
When I entered her room, I introduced myself and asked her if she was having any pain before my morning assessment. She rated her pain 7 but when I notified the RN, she said her pain med will be due in the next hour. I put down the time for the next pain medication on the white board for the patient & RNs to see. Responsible stewardship: During my shift, I cared for my patient and also educated her on some of her prescribed medication. In addition, I educated her about the use of nonpharmacological intervention (hot packs, changing position) for pain relief since pain was her major challenge. As a responsible stewardship, I used the skills I acquired in school to take care of God’s creature

4. What clinical skills were you able to perform?

Vital signs, intake and output, I removed IV line of another patient, patient education.

5. How did your patient’s condition compare with the textbook picture?
The patient presented on admission same symptoms and causes of her disease compared to my reference for the pathophysiology of her disease.
