Patient Interventions Reflection

Decent Essays
From midterm to current my CI allowed me to handle more of her caseload, observe online documentation, and weekly progress reports on her patients. She encouraged questions, and thoroughly explained why certain treatments and activities were better than others. She allowed for open communication of patient interventions, which made me feel comfortable enough to take on more of her case load and contributed to my visual, and hands on style of learning.

From my initial visit to current my CI has ensured my environment promotes growth at my clinical site. My CI speaks to me in a clear and concise manner, allows for time to discuss patient interventions, and most recently has allowed me to handle responsibilities that are in my scope of my knowledge

The fact that Meadow Hill Hospital is such a large facility with 400 members working there and having to be in charge of 211 beds is a big task. Being one of the leading hospitals should say a lot and having the finances to support its staff and IT department is very clever. The data and software all seems to be up to date and running smoothly in order for the hospital to perform their duties. Having a clear communication with all of the staff including nurses, pharmacy, laboratories, imaging and risk management. There might be some limitation due to not having the IT Governance in place or the fact that CIO is not available which will provide some limitation when it comes to being on top of the data and software used and ran in the hospital…

If a nursing intervention follows the nursing process, e.g., assess, diagnose, plan, implement, and evaluate – it demonstrates the clinical reasoning that supports the patient care. As cited by Alfaro-Lefevre (2012) Skip the principles of assessment and diagnosis and it’s easy to jump to conclusions, miss risks, and give care based on assumptions rather than evidence. Assessment errors and omissions are a major cause of adverse outcomes. If you fail to plan before implementation, the risk of adverse outcomes also increases. Skip evaluation, and reflective nursing practice (not to mention patient safety) goes out the door (p. 1).…

I think a cross-functional PI team should be convened to address the patient type coding issues between the ED and the cath lab. A cross-functional team is necessary because the system in need of improvement involves more than one department. The team should be made up of individuals from the catheter lab to specifically include a scribe and a lab manager or supervisor; the Emergency Department including an admitting clerk and their supervisor; Administration to include someone from operations like the Medical Director (Chief Clinical Officer); and someone from Human Resources, specifically a manager in the employee and labor relations department. Based on research, it would also be beneficial to not only include a member of the Quality Management Department but also make that person the team leader because experience leading similar performance improvement projects increases the…

However, sceptics have questioned how the concept is sometimes interpreted, arguing that there is unclear logical and pragmatic examination of its value in professional development. Therefore, developing a reflective report is an undertaking that needs to be approached with caution, more so if individual reflection and critical thinking are to be rooted. The purpose of this reflective report is to reflect on the collaborative boat bus project performance and the experiences of working within a multidisciplinary group. The report will give consideration to the feedback we obtained from the client and from Adam Smith (tutor) and how we then applied this feedback to feed in the report writing. An iterative model, meaning learning through participating, somewhat adapted to the one in Bulman and Schultz (2013)’s ‘Reflective Practice in Nursing p232’, Gibbs (1988)’s reflective cycle explains the significance of the evaluation process, especially building on negative as well as positive feedback.…

The name of the core competency is Apply quality improvement. For the organization strategic plan, C.D and her staff are planning to improve the level of care provided by health care professionals as “proof of better documentation in evidence-based preventative measures.” What are some of the evidence required by your organization C.D? I am glad you brought up that faculty needs to be “competent” in Evidence Based Practice (EBP). I had the opportunity to work for a college as a Clinical Adjunct Instructor (CAI). In doing so, CAI were required to monitor, teach, and be available for questions in the clinical lab prior to going to sites. I had a hard time adjusting to the skills check off list due to some of the skills were not up to date such as Foley catheter insertions. When I brought this to their attention including providing EBP literature.…

Explain your understanding of the rationale for this EBP model. My time as a case manager, in a harm reduction program for homeless veterans has allowed me to have weekly sessions with a client for over two months now. The client is an Army vet with a history of substance abuse secondary to Post-Traumatic Stress Disorder (PTSD). In my effort to asses the client’s mental health on a weekly basis, I took notice in how the Feedback Informed Intervention (FIT), provided a unique opportunity for me and my client. Boswell, Kraus, Miller, & Lambert (2013) says there 's a moment in time when the patient and provider can look through a window together which the therapist and patient can catch glimpses into a unique world-view (Boswell, Kraus, Miller,…

The purpose of this change proposal is an intervention to save the nurses from themselves of self destruction and the healthcare system (Noben et al., 2015). By adding certain features in the room as in massage chairs, focus points, music, and relaxing techniques could be added features for a calming effect. Having a meditation room will allow a balance for the nurses’ mental health ultimately improving patient care satisfaction and issues with staff shortages. With being aware of the mental health of the staff members could decrease turn- over rates, call offs, improve health of the staff, an increase in productivity and have a considerate amount of cost saving abilities. If a nurse is able to determine that they need a break or assistance…

Reflecting back on the course of Interventions II, the coursework and in-residence portion had given me a better insight in learning how to define my personal and professional identity. The implementation of a “mock” clinical therapy sessions during the in-residence portion had been a challenge in the beginning due to the lack of having any type of background in conducting a therapy session. These therapy sessions provided the needed feedback from the instructor and my peers to gain a better understanding my theoretical orientations and areas in which I can work on to become a more competent therapist. The coursework that was provided through this course also assisted in having a better knowledge base for my future practice based on empirically…
