For nurses to be able to get hold of this problems and make our patients happy, we have to understand their level of education and their learning style to come up with a teaching method based on nurses assessment of the education and learning style. When preparing a care plan for our patients, we should make teaching according to individual culture diversity, learning style and low health literacy aaand this help tobring other health care team on board like social workers, case manager, and dietician. Primary and secondary prevention. In Primary health prevention, the nurses should use skilled competence which are, collaboration, communication, assessment, and an advocate for the patient, and pays attension to both patient and the environment. Primary prevention is focused on the pursueing of segments of the population at risk and then providing informative programs(Townsend, 2011 p. 710 - 11). In this category where black falls within minority, diabetes is common among the blacks, primary education on nutrition, both in school and at home, exercise, teaching them not to spent all their day in front of the computer and avoid too much sugar intake are very important starting from elementary. His son was the interpreter because patient does not speak English. Among the medications he was taking was lasix 40mg p.o. to pull excess fluid from the lungs and feet and legs. He was not happy with the frequency of urination. After few days he was discharged with few more days of lasix till his next one week visit, getting home, he had the opportunity of stopping the lasix and some other medication on his own despite the education on the benefits of the medications, a week later, instead of follow up doctor visit, he was brought back for
