D-The patient AWOL yesterday due to the need of going to the hospital. The patient provided this writer of her discharge summary from St. Francis as the patient was diagnosed with Fibroids and a fever. The patient is prescribed with ibuprofen. This writer informed the patient that she will not be dosed immediately as any hospitalization will have to address with the clinic's TEAM and the patient has to be medically assessed. The patient is willing to comply with the term. Please note, the patient arrived to the clinic with her granddaughter, Riley (1 years old). This writer made copies of the patient discharge summary, distributed in TEAM staff members folder, and had the patient signed an ROI for this writer to obtain the entire medical …show more content…
This writer listened to the patient as she expressed herself and validated her feelings. This writer questioned the patient as to why she decided to participate at a methadone clinic and her expectation. Then this writer address positive views on the patient recovery, which clearly had a smile on the patient face. It seems the patient needs reassurance about her recovery and how she's doing. In addition, it also seems that the patient does not give herself credit enough. Furthermore, the patient also discussed about not doing any mental health services as she wants to focus on her health because yesterday, according to the patient, " I was in so much in pain....I couldn't stay.....I asked Frankie what's wrong with me and to take me to the hospital....didn't know this was going happen...so much going on." This writer says, " Elaner, take of care your health because your health comes first. Next month, is a new month....we will address the mental health piece at a later session, but right now, do not make irrational decision to leave the clinic as you have been doing well....Look at your UDS result ( this writer turned the computer screen)....You're doing it....this is all you. Take care of your health and update
