This is 41 year old African American male who is here for BP follow-up. Patient is requesting medication refill and PSA evaluation. Patient denies chest pain, SOB, N/V/D, or fever. Patient is very talkative and in a great mood. Patient smokes one pack per year for past 20 years. Also has history of cocaine obtuse, clean for past 4 months.
