The store housed approximately 100 SKUs that encompassed various sizes or products. Within the drugstore there was an entire shelf along the aisle, with an approximate size of 6 feet by 20 feet that displayed the various products. I am a registered pharmacy technician and have practised in community pharmacy for almost 6 years in the UK. The services that the pharmacy was providing were like where I had previously worked and all the standard operating procedures were similar. However, during the placement, I could see the pharmacist’s point of view rather than a technician and how the role of the pharmacist is different from that of a pharmacy technician. The pharmacist’s role was more focused on the clinical aspect, rather than just dispensing and ordering medication. I learnt how to recruit patients who are eligible for Medicines Use Review (MUR) and New Medicines Service (NMS).…

This was a main difference I noticed. A retail pharmacist works more independently and is not consulted as often. A clinical pharmacist in a hospital setting works as part of a team. I prefer working in a group so being a clinical pharmacist would be my first choice. Apothecaries, people who sold and distributed medicine, were prime factors of the Elizabethan Era, as well as the Four Humors. Even though many people could have thought the Apothecaries were made up ideations of legitimate doctors, they played one of the greatest roles in the medical fields during the Elizabethan Era, because their practices are still relevant, they discovered herbal cures and chemical breakthroughs that helped save lives, and paved the way for pharmacies centuries later, alongside the beliefs of the Humors. During the Elizabethan Era, Apothecaries and the Four Humors went hand in hand. Medication Reconciliation in the Hospital Setting The transition of patients from an acute care setting to a home setting is often challenging and stressful. It can be complex for the patient to understand the instructions for discharge and, more importantly, it can be challenging and dangerous when it comes to ensuring the patient understands the medication reconciliation process. Successful transition to home is multifaceted and depends partially on an accurate and complete overview of all medications with the patient. This is an imperative safety measure across the continuum of care (Ruggiero, Smith, Copeland, Boxer, 2015).…
