Purpose: to assist families and facilities for improvement when choosing a skilled nursing facility
Focus population: Patient families, patients, and facility administrators
Local Organizations that may be interested in the survey results include: Ga dept of Community Health,
Centers for Disease Control and Prevention, Occupational Safety and Health Administration
1. There is enough staff to handle the workload
2. Staff follows standard procedures to care for residents
3. Staff are trained often and thoroughly
4. Staff report their mistakes and are coached on improvements
5. Residents needs are met before and after change of shifts
6. Reports from residents and staff are taken care of
