SC received a call from Pa’s informal CG/dtr Tina Carroll on 3/30/2016. Tina reported that the Pa fell at the front door and hit het bottom. Tina called 911 and the Pa was transported by ambulance the Penn Presbyterian Medical Center. The Pa was examined and treated in the ER and was placed under observation. Tina reported no broken bone but that the Pa is very sore and shaken-up.…
Mrs. Schaefer was eventually transferred to the rehabilitation floor (July 16) and I made contact with Mrs. Schaefer, the nurses and spoke with the case manager assigned Victoria Fisher. Ms. Fischer provided me with her contact information and I forward a signed medical release for medical records. I inquired about team meetings and I was informed that they occur every Thursday and one was scheduled for July 20 at 9 a.m. I attended this and the team was still in the process of assessing and evaluating her.
