The patient appeared to be emotional and upset as she detailed about Nurse Melissa and Jennifer at laughing at her about her need for a dose increase since her dose was dropped previously due to the paitent's AWOL as her dose was decrease of 10% of 130mgs. The patient expressed that she felt humiliated and Nurse Aileen intervene to help the patient as the patient says, "She is always nice to the clients." The patient requested to file a grievance against two nurses as to how she was treated. This writer informs the patient that her request was submitted to Nursing on the 15th of May and will immediately address it with the APRN about the status. This writer informs the patient that this writer submitted the request prematurely as predicted; however, the APRN will increase the patient dose tomorrow. In addition, this writer turned the screen for the patient to review her fentanyl results, including her other positive results for the month of February, March, and April. The patient in return says, " We discuss this before. I am already aware of it....Charlene, I am trying, but I feeling unstable right now and I need to go up on my dose." This writer understand the patient frustration about the need for a dose increase, but mentioned about attending group as part of clinical intervention. The patient then says, " C'mon, I just lost my job (patient started to cry). I am out here looking for a job right now, every day. I do not have time to go to the group sessions." This writer then says, " But, you cannot continue to use illicit drugs, Erica. What do you want from your recovery because it's one thing to say to get clean, but you need to make changes." The patient was crying in this writer's office and then evenually agreed to attend the Fresh Start group as she is unable to attend the cocaine group due to conflicts with Facilitator Cherron, whom groups are held on Thursdays and the Fridays group hours are too early as the patient have to drop her son off to school.
