Client History and Primary Concept
My assigned patient was M. P, a 51 years old male that was admitted to the HCMC psych unit on the 09/09/2015, after he was reported as an unwanted person at an apartment in uptown. He was brought in by Minneapolis police on transfer hold. Per police, the patient is under civil commitment and listed as a missing person in Washington County. (HCMC note review, September 2015). The patient’s medical diagnoses are: bipolar disorder, schizoaffective disorder, and chronic airway obstruction. Besides his medical diagnosis, he had acute anxiety, high nicotine consumption, and had sustained blisters on his left arm and feet for walking excessively. Stress is a pattern of physiologic and psychologic responses determined through cognitive appraisal. (Guiddens, 2013). the patient described many life event that he perceived as taxing such as: he mom passed away, he dad doesn’t talk to him because his little sister lies to his dad about him, he has 3 children with 3 different moms that he rarely see, he lost his job and cannot pay charge support any more, and he just broke up with his girlfriend. The coping mechanism used by the patient was nicotine and alcohol consumption. The patient ceaselessly complained about anxiety and was requesting his PRN for anxiety every 4 hours. According to his medical diagnosis, he is cognitively impaired, which affect his ability to cope with stress and anxiety. It also affects how he appraises …show more content…
It is important to encourage patients to participate in social activities because they can learn new things and get their mind away from visual hallucinations, auditory hallucinations, and other symptoms. I will also plan and implement preventive measures with every patient and family that I encounter, such as
