“ Case management is a second strategy to improve patient care and reduce hospital costs through coordination of care p. 254.” The role of a case manager is coordinating care and making goals from the time of admission to the time of discharge. The goal is to control health care cost and to avoid fragmentation. Because the case managers role are extensive, they require an advance practice degree or advanced training. Some of the roles nurses have in the case management process is coordinating care, advocating, enhancing care quality, and facilitating continuity of care.
