Right ankle and foot I was unable to check for range of motion due to diagnosis of fracture of ankle and foot. Left, foot and ankle appears normal. No abnormalities or deformities in upper. Full range of motion in upper extremities. Neurological- He is alert, orientated to person, place and time.

This patient has been diagnosed at Hackensack University Medical Center on 12/12/16 an Emergency room visit. The follow up care and instructions I included the assessment and the plan of care to the patient with the approval of my preceptor.
