Monday, November 9th: Today I was at Moses Cone hospital in both a nurse leader and students nurse role. This clinical day was our 12-hour shift day. I was co-leader with Samantha, and we shared a case where a patient was admitted for a spinal injury with a host of other previous medical issues, including confusion. It was today in my role as student leader that I felt the most comfortable fulfilling my duties. Sam and I worked a long time the day before making sure that the patient assignments fit our peers requests and capabilities. After the initial assignments, and gaining more information from the patients chart including MAR, and the nurses, we made some adjustments to the assignments. When we first went to get report from the night nurse, she asked us “Why do you want this patient?” We were caught of guard by her question and just explained that the case seemed detailed and interesting and she replied, “okay, well I guess if there are two of you, then it will be okay.” Our patient required a lot of care, but working together made it a lot easier than it would be alone. We had originally planned to have one of us care for the patient the first half of the shift and then switch off, but after report, we decided it would be a better idea to take it on together. Having this one patient by ourselves would have kept us busy the whole day. Our patient had not put out a lot of urine so a bladder scan was done, and showed the patient had 427 mL of fluid in her bladder, and required a catheter. The physician was called to determine if they should do an in-and-out cath or a Foley catheter. In the end, the Foley catheter won because it reduced the risk for infection and because our patient was incontinent of urine. Samantha got to insert a Foley catheter, which was neat to watch. She did a great job! I hope when I get to perform this skill that I will do a good job as
