A review of the medical records indicates that she suffer from multiple medical illness which include PMH of hip replacement, chronic arthritis, chronic spinal stenosis, chronic lumbar radiculopathy, chronic stable hypothyroid, chronic COPD- she is a smoker and refused smoking cessation. She has a PMH of colon and cervical cancer.
At today’s visit she is accompanied by her caregiver. The patient is awake, alert ad oriented. She complains of pain “all over, worse in thigh and lower back” severity 10/10, unable to describe her pain, some days her pain is worse than other.
