The client’s history is notable for having thyroid disease and sinus infection. Mrs. M’s earliest medical problem was a skin disorder during childhood, in which she was no longer diagnosed with. Later on in adulthood, she developed a thyroid disease that lead to a hysterectomy. According to the records, Mrs. M’s was born normally at nine months. The client did not have any problems during labor or delivery, and she was not exposed to any toxins or teratogenic agents prenatally. Mrs. M’s crawled at six months, talk single words at ten months, walked at one year, and talk in sentences at two years old.
