Mr. Trimmer is a 29 year old male who presented to the ED via LEO following an incident at Walmart where he gave a cashier a note informing them he was going to harm himself with intent to cause pathological results. Mr. Trimmer reports to ED staff he was going to stab himself in the neck with a screwdriver. At the time of the assessment Mr. Trimmer appear guarded and irritable. He reports today sitting in the bathroom at Walmart for several hours, then going to a cashier at Walmart with a note informing them he had a plan to stab himself in the neck with a screwdriver. He states, "I don't give a fuck anymore." Mr. Trimmer reports being "jumped" 5 days ago and having all his belonging taken from him. He reports relational issues, homelessness, substance dependence as stressors contributing to his distress. He expressed depressive symptoms as feelings of hopelessness, tearfulness, irritable, anger, isolation, and sadness.Mr. Trimmer currently endorses suicidal ideation and symptoms of psychosis. He states, "I hear suicidal and homicidal voices." Mr. Trimmer expressed if he leaves tonight he will "find a way" to harm himself. Mr. Trimmer currently …show more content…
Trimmer most recent UDS he tested positive cocaine and marijuana. He reports being on a 2 week binge of "Crack", alcohol, and meth. He reports using meth, cocaine, marijuana, and alcohol for 10 years. Mr. Trimmer reports he last used "Crack" Wednesday. He reports experiences withdrawal symptoms as tremors, chill, and diarrhea. He reports no receiving no services. He reports just getting out of Durham Rescue.

Due to an increase in depressive symptoms, suicidal ideation with a plan, history of attempts, patient does meet criteria for IVC and inpatient hospitalization. TACT consulted with Dr. Osborne, it was recommended to refer for inpatient hospitalization for safety and stabilization. TACT assisted the ED doctor in completing IVC paperwork. TACT will search for appropriate placement.
