DOI: 10/4/2007. Patient is a 43-year-old male upholsterer who sustained a work-related injury to his left knee when he was struck on the table.
Per the narrative report on medication management dated 03/11/16, the patient presented for medication management for persistent symptoms of depression, anxiety and stress-related medical complaints.
It should be noted relevant to multiple medications that there have not been any significant side effects or negative interactions relevant to these medications. It should also be noted that the medications all interact to improve anxiety, depression, confusion, emotional control and stress-intensified medical complaints.
It would be unrealistic to presume that each medication works alone on one or two target symptoms.

Anxiety cluster symptoms include excessive worry, restlessness, jumpiness, tension, agitation, panic attacks, feeling “keyed up” or on edge, inability to relax, pressure, shaking, chest pain, palpitations, nausea and shortness of breath. Post traumatic stress disorder cluster symptoms include disturbing memories and reliving of the trauma. Altered perception cluster complains include hearing voices, suspicion, fear of being followed and fear of being monitored. Stress related medical cluster included tension headache, muscle tension, temporomandibular joint/jaw clenching, increased pain, erectile dysfunction, peptic acid reaction, abdominal pain/cramping and constipation. Improvement in symptoms and functions include being able to concentrate better, comprehend television, gets along better, less time in bed, goes out more, less fatigued, less depressed, less yelling, less nervous and less
