After the incident Duke University Hospital made a policy that three crucial members of the transplant team, the transplant surgeon, the transplant coordinator, and the doctors who harvest the organs were to confirm the blood type of the recipient and the donor. These are just a couple of the cases of the death caused by medical error, these cases happened every day.

Other form is the written form where writing is not clear enough, and reader is assuming what is written, or sometime right dose of medications is not correctly placed in order and the mistake is not caught. For example if the physician write an order for dose of .5mg it is very easy to interpret that as a 5mg dose. If we taking in considerations of the pediatric dosage this mistake could be deadly. There are so many medications that have a similar names and is very easy to mistake one for another. For the past few years there are some abbreviations that are prohibited in use in the health care field, since many health care personal start making their own abbreviations. But one of the most important mistakes that people make in communications is the no communications at all. There was the case that happened at the Duke University Hospital where an engineering team were replacing the hydraulic fluid in the elevators and did not have a proper containers to dispose the used hydraulic fluid, but used an empty detergent barrels in which the hospital receives a surgical …show more content…
Effective communications between healthcare providers, patients and their families should be done in effective ways that information is clear and correctly transmitted from sender to receiver without any misunderstanding. Respect patients in their values and their cultural belief. Each team member of in the Healthcare Organization is responsible to provide an effective communications and work as a team member to provide a best possible care to the patients. Developing and implementing an effective communications throughout Healthcare Organization is to encourage collaborations and prevent errors. In the Healthcare Organization it is important to address and resolve any issues no matter how small they seem to be, that may have a high risk of miscommunications and errors.
