Mindy is a 31yo, G2 P1001, who was seen for an ultrasound evaluation for dating and fetal anatomy assessment. This is the patient’s first ultrasound. Overall, she denies any major medical disorders. She reportedly is hypothyroid but is currently not on treatment. I am uncertain as to whether or not she has had a thyroid study performed. She states that her primary issue has been heartburn and was told to try some TUMS. She also has had some nausea and is on Unisom and vitamin B6. Based on her prepregnancy weight of 125 lb, she has gained 6 lb. Her vital signs were normal today. On today’s assessment, she does have positive fetal movement.

By dates using her LMP of 05/16/17, she has an EDD of 02/20/18 placing her at 02/20/18 placing
