Per the AME report dated 3/9/15, the patient has reached maximum medical improvement and he should have access to future medical care which includes refills of pain medication on an as-needed basis, access to short courses of physical therapy and acupuncture. The need for further injections and surgery in the future cannot be ruled out. It has been one year since he has been to this office.
He is status post initial repair of laceration followed by a second surgery with Dr. Chen for digital nerve exploration and neuroma excision in 2015. He states he has persistent pain swelling numbness and tingling of the right index finger.
Examination of the right index finger demonstrates a healed longitudinal laceration with tenderness to palpation over the course of the laceration. Sensation is decreased over the radial digital nerve.
