Per medical report dated 02/27/2015, patient’s medications are gabapentin 300mg, Klonopin 1mg, Lidocaine 5% patch, lithium carbonate 300mg, naproxen 500mg, and Saphris 5mg.

Gait is slow and guarded.
Diagnoses are displacement of lumbar intervertebral disc without myelopathy, moderate bipolar disorder, degeneration of intervertebral disc, coccyx disorder, and degeneration of lumbar intervertebral disc.
Treatment plan includes follow up with Dr. Weiss, follow-up visit in 2-3 months, trial of nabumetone for pre-menstrual exacerbation of low back and right lower extremity pain, alternate with Naproxen as needed, and continuation with Naproxen, gabapentin and lidocaine patches. All medications reduce pain by half and allow for the patient to continue daily activities.
He was given prescription for Lidocaine 5% patch 3 patches to be applied to the affected area for 12 hours on and 12 hours off #90, naproxen 500 mg 1 tablet twice daily #60, gabapentin 300 mg 2 capsule sin the morning and bedtime (to increase 1 capsule each week to titrate) #120 and nabumetone 750 mg 1 tablet twice daily #60, all with 2 refills.
