Left Knee Injury: A Case Study

DOI: 5/28/2013. Patient is a 54-year-old female apparel processor who sustained a left knee injury when she was going upstairs and her knee popped causing sharp pain. As per Omni, the patient was diagnosed with left knee patellar chondromalacia, medial synovial plica and torn medial meniscus. Patient is status post left knee arthroscopic partial medial meniscectomy, patella chondroplasty, medial femoral condyle chondroplasty and excision of synovial medial plica in 10/18/2013. IW was deemed P & S on 06/15/15 with 1% whole person impairment rating.
Per the PT note dated 02/05/14, the patient has attended 12 sessions for the left knee.
Based on the initial orthopedic comprehensive report dated 09/16/16, the patient complains of constant left

It was opined that the IW has not reached maximum medical improvement (MMI).
She was given a prescription for Celebrex 200 mg #30 1 capsule daily for inflammation, Protonix 20 mg #60 1 tablet twice daily for gastritis secondary to nonsteroidal anti-inflammatory drug (NSAID) intake, Ultram ER 150 mg #60 1 tablet twice a day for pain, Fexmid 7.5 mg #120 1 tablet three times daily to relax muscles and Neurontin 300 mg, #90, take one tablet twice daily for nerve pain.
Treatment recommendation includes ultrasound guided Synvisc injection to the left knee, MRI arthrogram of the left knee, and PT 2 times a week for 6 weeks to focus on strength training, increasing range of motion and decreasing pain. Follow-up is in 6 weeks.
Attached is a copy of the MRI of the left knee dated 07/24/13.
Per verification from the provider’s office, the IW has not had a steroid injection to the left knee.

Current request is for 18 Physical Therapy Visits for the Left Knee between 9/27/2016 and 11/26/2016.
