He continues to have deceased short-term memory ability but unchanged from when last seen by me. He has mildly decreased concentration ability. He continues to have construction dyspraxia, but no other dyspraxia, but no other higher cortical function deficits. He has mild to moderate decreased executive cognitive ability. He has a mildly masked appearance of his face at rest. His face continues to symmetric at rest with good strength throughout his face. The remaining of his cranial nerve examination was within normal limits. He has good strength of his extremities. He has mild cogwheel rigidity of both wrists and minimal cogwheel rigidity of both elbows. He has minimally increased tone of his lower extremities, not true cogwheel rigidity. No tremor of his hands. He has mild bradykinesia of movement of his extremities as well as of standing. He is able to stand on his own, though.
