Medical History:
Jordan explained that he has been hospitalized on several occasions for strep throat and broken bones. He noted that his broken bones were typically a result from riding dirt bikes, four-wheelers, and jumping off of things. Jordan related that his PCP is Dr. Anthony Ubert and has not had a check-up since November/December. Jordan acknowledged that his Tryclycerides were elevated and he failed to return for blood work, noting that he was unconcerned about his health.

Medication:
Jordan reported that Dr. Mehmet Ulus, a Psychiatrist at CCC, prescribed him Perphenazine 2 mg tablet twice a day and Bupropion (i.e. Welbutrin) 150 mg tablet once a day. Jordan disclosed that he has not always been compliant with his medication (e.g., not taking his medication or drinking alcohol while taking his medication).
Legal Involvement:
Jordan denied any history of arrests or legal issues. Conversely, he noted that he has been cited twice (e.g., hunting violation and failure to follow directions written on a sign). Jordan reported that he was upset and denied any responsibility for his actions.

Drug and Alcohol:
Jordan endorsed that he typically consumes tree beer in one sitting, approximately three times a week. Historically, Jordan would engage in binge drinking to cope with affective instability and interpersonal discord, which has persisted all weekend long (e.g., from Friday to Sunday). Although he could not articulate the amount of drinks he consumed, Jordan indicated that he

As a measure of current general intellectual functioning, the KBIT-2 was administered and yielded a Verbal Score = 86±5; a Nonverbal Score = 55±5; and an IQ Composite Score = 66±4. The composite score falls within the very low or borderline educational classification at the 01st percentile rank per national age based norms and needs to be viewed with caution. Due to Hunter’s struggles with focus and difficulty attending to auditory instruction, his nonverbal score is felt to be an under representation of his actual performance. Repeated testing within the near future with this or a similarly constructed psychometric instrument would probably find Hunter scoring higher than an IQ band of 60 - 74. Analysis of KBIT-2 scores found Hunter demonstrating an unbalanced skill set between his verbal skills and his spatial, visually based skills.…
