A review of the medical records indicates that he was recently admitted to MMHS for altered mental status and was diagnosed with CVA( intracerebral hemorrhage). He was sent to SNF for rehab after his hospitalization. Previous to his CVA he was living at home. He also suffers from chronic HTN, chronic first-degree AV block and chronic dyslipidemia. He also has a history of colon cancer and prostate cancer.
At today’s visit he is accompanied by his daughter who met me at the facility. He is awake and lethargic. He appears short of breath, he is wearing his oxygen. The daughter reports that the patient did not want to be hospitalized when he had the intracerebral hemorrhage but was taken to the hospital without her knowledge. She states that
