The conversation often returns to the fact that no human can 100% of the time predict the outcome of the patient - because there are the scenarios where a patient in a coma miraculously returns to leading an independent life. While this situation can occur, it is very unlikely. The thought of having someone else make medical decisions on my behalf because I am incapable makes me uneasy. While I have seen situations where medical paternalism seems necessary, specifically with mental health patients, I have seen scenarios where medical paternalism…
Introduction A common issue in all health care fields is adherence to treatment. It is difficult for individuals to change their behaviours and to maintain those behaviours. Every person has their own perspective on their health and makes decisions about their health in different ways. Some individuals deny that they have a health problem, some are reluctant to manage their health, and others follow advice from practitioners without question.…
The Healthy People 2020 program is a strategy that involves recognizing nationwide health concerns and increasing public awareness and understanding of the determinants of health, disease, and disability and the opportunities for progress. Cardiovascular health form part of the items of interest which the health communities lay a great deal of emphasis on. The importance of patient participation in healthcare decision making is extremely important it keeps patients involved with the care they are receiving, it empowers them, and improves services and health outcomes. Health care professionals such as doctor-patient relationship, recognition of patient’s knowledge, allocation of sufficient time for participation, and also factors related to patients such as having knowledge, physical and cognitive ability, and emotional connections, beliefs, values and their experiences in relation to health services. Shared decision making aims to support patients to articulate their understanding of their condition and what they want to achieve from treatment, inform patients about their condition and the pros…
A conflict between a physician and patient on the course of treatment is a common topic that arises in bioethical conversation. This case explores the topic of paternalistic choices dealing with patient autonomy and when it is ethically reasonable for a physician to intervene and decide as to whether an individual is competent to make decisions about their own care. The patient in the case, Mr. Howe was asked to make a lifesaving medical decision while in duress and not fully understanding the procedure and the potential outcome if he refused. In this case I believe the physician made the correct decision to intubate Mr. Howe against his explicit instructions not to. The physician made a determination that the patient was not accurately expressing his wishes as if he was of sound mind.…
A Duty to Care: The Role of a Nurse in Circumstances of Patient Nonadherence Health care professionals are all too often aware of the lifestyle choices that have led the patient to seek treatment in their care. However, the implications of these lifestyle choices and issues that can arise from nonadherence are often downplayed or simply disregarded by the patients despite the urgency of the matter being expressed by the health care professionals. In other words, sometimes, the patients refuse to alter their lifestyle for the preservation of their own health. This can be frustrating, to say the very least, but it does not, under any circumstances, dismiss the health care professionals from their duty of care (Laken, 1983).…

Valeria Johnson coming to me today made me realized that although we work together and have a good working relationship she was willing after our session to tell me she would work with me in this health concern. Most people for the most part do not like being told what to do and it gets worse if it is about their bodies and how they should live. What does MI look like? It involves asking questions and responding to patients' comments in a way that validates their experiences, helps them develop reasons for making health behavior changes, and acknowledges patients' control of the decision to change. So a better way to get to help her was introduced motivational interviewing (MI) and by using this method I was providing advice and feedback.…
The purpose of this change proposal is an intervention to save the nurses from themselves of self destruction and the healthcare system (Noben et al., 2015). By adding certain features in the room as in massage chairs, focus points, music, and relaxing techniques could be added features for a calming effect. Having a meditation room will allow a balance for the nurses’ mental health ultimately improving patient care satisfaction and issues with staff shortages. With being aware of the mental health of the staff members could decrease turn- over rates, call offs, improve health of the staff, an increase in productivity and have a considerate amount of cost saving abilities. If a nurse is able to determine that they need a break or assistance…

The model fits a patient education approach to resolving the problem with poor patient compliance. Using HPM, the nurse takes on the role of educator and performs the initial task of assessing the patient’s perception of health in order to develop individual plans of care. This allows the nurse to create a comprehensive plan tailored to the individual patient so that the patient is able to achieve optimal self-care and disease prevention. By performing a thorough assessment and constructing a highly individualized plan of care, the nurse is able to focus on the patient’s strengths and identify weaknesses that prevent the patient from achieving compliance, and to take responsibility for their role in their own health and wellness. Empowering the patients will motivate them to make the necessary changes in behavior as well as their environment in order to promote better health outcomes (Petiprin,…
We are their teachers, advocates, friends, and confidants. By acting as all of these, we are able to have an influence in our patient’s feelings towards medicine and the medical world. It is my belief that with this influence, we should be providing our patients with the tools they need to promote a healthy wellbeing for themselves. As nurses, we should be advocating for preventive measures to keep our patients healthy, such as yearly physicals, smoking cessation, and methods of reducing and dealing with life’s stressors in a positive manner. It is my belief that if we provide these outlets to our patients, we are giving them the keys to a much healthier lifestyle, in which they will likely reduce the amount of illness they may encounter.…

According to the College of Registered Nurses of British Columbia (CRNBC, 2014) nurses make a clinical decision every 30 seconds, or approximately 1500 per shift. What is a clinical decision? The answer to that question varies among the experts and practitioners but the terms clinical judgment, reasoning, and decision making have been used interchangeably in attempt to better articulate the process of clinical decision making (CRNBC, 2014).…
