This is 39 year old AAM. Patient is here for his HTN, hyperlipidemia, and DM follow up. Patient is also here for his annual fasting labs; LIPID; CMP; TSH. Patient is taking all his medications as prescribed, walking daily, and eating a healthier diet. Patient is a JBS resident and currently followed by JBS psych.
